At this visit you can discuss your child’s behavior and strategies that may improve those behavioral issues you are having. If these behaviors are possibly caused by a disorder such as Depression or ADHD, this visit can also begin the evaluation for mental health disorders.

To speed your first visit, bring in the below forms and records completed. If you have concerns for ADHD or Depression, filling these forms out will save you a visit.

Before your visit

  • Valid Parent ID

  • Insurance Card (update)

  • Consent for Treatment

    • If you are having a non-guardian take your child to clinic you’ll need this form filled out for that person. If not, this form is not required.

  • ADHD Concerns?

    • Vanderbilt Assessment Parent

      • This form should be filled out by at least 1 parent and helps diagnose ADHD and other disorders that mimic it. Both Parent and teacher forms are required for diagnosis.

    • Vanderbilt Assessment Teacher

      • This form should be filled out by 1 teacher, preferably that sees your child perform in multiple areas (math, reading, etc.) Both Parent and teacher forms are required for diagnosis.
